All receipts were produced as PDF files and emailed.  If you did not receive your receipt, it may be for one of these reasons:

• Some emails were rejected by the receiving mail servers for a number of different reasons (invalid addresses, full mailboxes, etc);
• You should check your junk mail folder and your anti-virus quarantine folder in case the email and/or the attached PDF file were incorrectly moved there.

If you did not receive your receipt, please use our Contact form to request us to resend it.

NOTE:  We will not be sending out receipts for any of the clinics.  The confirmation page you received when you submitted your online registration is your receipt, and should be printed or saved at the time you register.
